LeConte Sports Medicine will hold pre-participation screenings for any Sevier County high school athletes who have not yet completed the screening on Thursday, May 27, 2010, 5 p.m. – 7 p.m. Thursday’s screenings will be held at Sevier County High School (enter through the main entrance to the High School). The cost for the screening is $10.
The pre-participation screening is required for any high school student participating in school athletics. Registration and HIPAA forms should be completed prior to arriving for your screening, and can be downloaded at www.lecontemedicalcenter.org/sportsforms.
Parent signature is required on these forms, so please complete prior to the screening time. May 27 is the last scheduled date that LeConte Sports Medicine will be hosting pre-participation screenings.
