LeConte Sports Medicine is gearing up for our annual pre-participation athletic screenings! All student athletes in Sevier County are required by the Tennessee Secondary Schools Athletic Association (TSSAA) to have a pre-participation screening in order to be eligible for tryouts or practices for the 2011-2012 school year. The TSSAA requires that these pre-participation screenings take place between May 1 – August 30 annually in order to participate in athletics in the coming school year.
LeConte Sports Medicine will hold pre-participation screenings for all high school student athletes in Sevier County on:
· Tuesday, May 3, 2011, 2 p.m. – 6 p.m., at Seymour High School
· Thursday, May 5, 2011, 5 p.m. – 7 p.m., at Sevier County High School.
The cost for the pre-participation screening is $10. Registration and HIPAA forms should be completed prior to arriving for your screening, and can be downloaded at www.lecontemedicalcenter.org/sportsforms .
A parent’s signature is required on these forms, and we ask that you please complete the forms and sign prior to the screening time. The student athlete should bring the forms with them to the screening site.
